  • Qualification for Scholarship consideration
  • Qualification for Community Service/Volunteer Hours
• I acknowledge that sessions may be video recorded or photographed for educational or promotional purposes.
  • If I object to being photographed or recorded, I will indicate it below in the Comments section.
• I understand that parents are welcome to attend and enjoy the complimentary meal.
• I will have the opportunity to take a picture with my parent(s)/guardian(s) on Day 2 of the seminar.
